This article considers why individuals take beyond their own needs at the cost of others. Within the context of negotiations, a story is employed in framing an examination of the advancement of self–interest over connections of interdependence and civic membership. Forces of natural predisposition and environment are juxtaposed to formulate an understanding of the struggle involved in acting ethically. Notions of distributive and procedural justice are discussed with respect to claims of American citizenship, religious right, and the centrality of happiness, (...) resulting in a call for the substantive justice of interest–based negotiation. Respect and dignity are central to moral life, and have a particular importance in health care settings such as the intensive care unit (ICU). We conducted 15 semistructured interviews with 21 participants during an ICU admission to explore the definition of, and specific behaviors that demonstrate, respect and dignity during treatment in the ICU. We transcribed interviews and conducted thematic qualitative analysis. Seven themes emerged that focused on what it means to be treated with respect and/or dignity: treated as a (...) person; Golden Rule; acknowledgement; treated as family/friend; treated as an individual; treated as important/valuable; and treated as equal. Participants described particular behaviors or actions that were considered related to demonstrating treatment with respect and dignity: listening; honesty/giving information; attention to body/modesty/appearance; caring/bedside manner; patient and family as an information source; attention to pain; and responsiveness. These behaviors provide a framework for improving experiences with care in the ICU. (shrink)

BackgroundThe COVID-19 pandemic has had a significant impact on the health system worldwide. The organ and tissue donation and transplantation (OTDT) system is no exception and has had to face ethical challenges related to the pandemic, such as risks of infection and resource allocation. In this setting, many Canadian transplant programs halted their activities during the first wave of the pandemic.MethodTo inform future ethical guidelines related to the COVID-19 pandemic or other public health emergencies of international concern, we conducted a (...) literature review to summarize the ethical issues.ResultsThis literature review identified three categories of ethical challenges. The first one describes the general ethical issues and challenges reported by OTDT organizations and transplantation programs, such as risks of COVID-19 transmission and infection to transplant recipients and healthcare professionals during the transplant process, risk of patient waitlist mortality or further resource strain where transplant procedures have been delayed or halted, and resource allocation. The second category describes ethical challenges related to informed consent in the context of uncertainty and virtual consent. Finally, the third category describes ethical issues related to organ allocation, such as social considerations in selecting transplant candidates.ConclusionThis literature review highlights the salient ethical issues related to OTDT during the current COVID-19 pandemic. As medical and scientific knowledge about COVID-19 increases, the uncertainties related to this disease will decrease and the associated ethical issues will continue to evolve. (shrink)

A novel Protocol Ethics Tool Kit (‘Ethics Tool Kit’) has been developed by a multi-stakeholder group of the Multi-Regional Clinical Trials Center of Brigham and Women9s Hospital and Harvard. The purpose of the Ethics Tool Kit is to facilitate effective recognition, consideration and deliberation of critical ethical issues in clinical trial protocols. The Ethics Tool Kit may be used by investigators and sponsors to develop a dedicated Ethics Section within a protocol to improve the consistency and transparency between clinical trial (...) protocols and research ethics committee reviews. It may also streamline ethics review and may facilitate and expedite the review process by anticipating the concerns of ethics committee reviewers. Specific attention was given to issues arising in multinational settings. With the use of this Tool Kit, researchers have the opportunity to address critical research ethics issues proactively, potentially speeding the time and easing the process to final protocol approval. Treating patients and their family members with respect and dignity is a broadly accepted goal of health care. The work presented in this article is part of a larger project aimed at better understanding what constitutes treatment with respect and dignity in the ICU to improve the care that patients and family members receive in this regard. Direct observation was selected as one of the methods to facilitate this understanding because it provides the opportunity to see and document what actually (...) occurs during encounters among patients, their families, and clinicians. This article reports seven major thematic domains and many subthemes that together create a detailed account of the interpersonal and environmental components of treatment with respect and dignity. Despite wide recognition of the importance of treating patients with respect and dignity, little is known about what constitutes treatment in this regard. The intensive care unit (ICU) is a unique setting that can pose specific threats to treatment with respect and dignity owing to the critical state of patients, stress and anxiety amongst patients and their family members, and the highly technical nature of the environment. In attempt to understand various stakeholders’ perspectives of treatment with respect and dignity, patients (...) and family members were interviewed, a wide range of health care professionals participated in focus groups, and third party observers took field notes of interactions in the ICU. This paper compares and contrasts the data that were generated using these different methods. In this study, we tested the feasibility of conducting quantitative assessments of patients’ experiences with care in the intensive care unit (ICU), in regard to treatment with respect and dignity. Patients completed the Patient Dignity Inventory, Collaborate, and selected domains from the Hospital Consumer Assessment of Health Providers and Systems Survey. Family members were additionally surveyed using the Family Satisfaction in ICU Care questionnaire. Overall, patients reported high levels of satisfaction in terms of nurses and doctors treating them with courtesy (...) and respect; however, physical aspects of care were reported to be more problematic. While this pilot study suggests some target areas for improving treatment with respect and dignity, the findings are limited since many patients were unable to participate in the survey. Future work should be directed at developing new measures that are easier to administer in this setting. (shrink)

Little is known about health care professionals’ perceptions regarding what it means to treat patients and families with respect and dignity in the intensive care unit (ICU) setting. To address this gap, we conducted nine focus groups with different types of health care professionals (attending physicians, residents/fellows, nurses, social workers, pastoral care, etc.) working in either a medical or surgical ICU within the same academic health system. We identified three major thematic domains, namely, intrapersonal (attitudes and beliefs), interpersonal (behaviors), and (...) system (contextual) factors that influence treatment with respect and dignity. Participants suggested strategies for improving treatment of patients and families in the ICU with respect and dignity, as well as the related need for enhancing respect among the multidisciplinary team of clinicians. Implementing these strategies will require innovative educational interventions and leadership. Future research should focus on the design and evaluation of such interventions on the quality of care. This essay is a reflection of the time I have spent working with Cardinal Newman’s archive at the Birmingham Oratory. I have had a chance to stop and carefully read his letters and diaries and to see Newman as a communicator extraordinaire! I suspect that the Cardinal would have had great command of today’s social media and communications technology. His laity could have been a wider and larger audience on a virtual level. Might this be an opportunity for (...) a sociological comparison of the great writers and speakers of an earlier time with those of us in today’s media-rich world? With whom else might Newman have interacted? We may never know with certainty, but it would be fun to imagine. (shrink)

Pancreatitis is a complex, progressively destructive inflammatory disorder. Alcohol was long thought to be the primary causative agent, but genetic contributions have been of interest since the discovery that rare PRSS1, CFTR and SPINK1 variants were associated with pancreatitis risk. We now report two associations at genome-wide significance identified and replicated at PRSS1-PRSS2 and X-linked CLDN2 through a two-stage genome-wide study. The PRSS1 variant likely affects disease susceptibility by altering expression of the primary trypsinogen gene. The CLDN2 risk allele is (...) associated with atypical localization of claudin-2 in pancreatic acinar cells. The homozygous CLDN2 genotype confers the greatest risk, and its alleles interact with alcohol consumption to amplify risk. Newman’s dramatic poem, “The Dream of Gerontius”, was set to music by Edward Elgar in 1900. This essay brings out the sympathy of mind and heart between poet and composer, and perhaps between them both and the listener of today, as well as the universality and depth of the human stake in some kind of personal and peopled life after death.

This article examines representations of motherhood in three transnational feminist films: Anayansi Prado's Maid in America (2004), Sabiha Sumar's Silent Waters (2002) and Marjane Satrapi's Persepolis (2007). While these films differ at the levels of genre and style as well as in terms of their production contexts, they each feature several scenes that engage the tension between distance and proximity, separation and unity – an always unresolved tension integral to the reproductive sphere. Each film also provides at least one close-up (...) shot of a mother's hands preparing food for her charge. Tracing Mary Ann Doane's explication of the close-up shot's engagement with the capitalist logic of the microcosm and macrocosm, I will show how the aforementioned films resignify what Doane identifies as the close-up's compensation for the loss of unity in modernity. The recurring image of a mother's labouring hands interrogates the false unity of the capitalist macrocosm, while also complicating the purportedly separate microcosm in which modern subject can touch and hold a given commodity. These films show that transnational mothers are simultaneously held within the immobile space of reproduction – owned and exploited by the larger structures that depend on their labour – while also actively recreating the cultures that render them invisible. I argue that a fruitful approach to exploring the significance of the abstract expressionist Barnett Newman’s body of work, understood as as an attempt to “paint the sublime,” is by appeal to Peircian phenomenology and the conception of “originativity” that it entails. By attending, in particular, to Peirce’s conception of “the firstness of thirdness,” I show how this “reasonable feeling” both signifies our “affinity” with the world with which we transact and, with specific respect to what happens when looking (...) at Newman’s paintings, explains why we are spurred to reflect upon the meaning of these experiences. In this way, “the firstness of thirdness” accounts for our recognition of a form of reasonableness that exceeds that exhibited in ordinary conceptualization. (shrink)

Modern knowledge of the grammarian Consentius’ De barbarismis et metaplasmis, a work valuable for the study of the Latin language, dates back to a relatively recent past: it was only in 1817 that its editio princeps was published by Ph.C. Buttmann, just a few years after the legal scholar A.W. Cramer came across a mention of the then unknown treatise in a ninth-century MS in the Bayerische Staatsbibliothek of Munich, numbered Clm 14666. Based on this solitary manuscript, H. Keil published (...) the short treatise in the fifth volume of his Grammatici Latini. With no little enthusiasm did W.M. Lindsay announce his unearthing of what, in his own words, had ‘long been a “desideratum”, a second authority’ for this text, in the MS F 15 III d at the Universitätsbibliothek Basel; this was followed by E.O. Winstedt's complete collation and M. Niedermann's critical edition. After about a century now there comes to light a third authority, surprisingly enough in a codex which has enjoyed such fame in the past decades that one might wonder how Consentius could have gone unnoticed in it for so long: this is the eleventh-century MS of Venice, Biblioteca Nazionale Marciana Lat. Z. 497, in which the De barbarismis et metaplasmis is contained on fols. 84vb 39 - 90va 39; moreover, a so-far-unnoticed quotation from it, together with one from Consentius’ De nomine et uerbo, is found on fol. 41vb 16–21 of the same manuscript in a famous grammatical florilegium. The codex, written in Romanesque minuscule and probably originating in Rome, is regarded as a handbook of liberal arts designed by Lawrence Archbishop of Amalfi, formerly a monk at Montecassino, thereafter a teacher in Florence and Rome, where he died in about 1049. Based on palaeographical evidence, F.L. Newton rightfully assumed as an exemplar for this codex a MS in Beneventan script, as some features can be detected that betray the scribal imitation of that typical South Italian script, namely the use of the distinctive abbreviation for eius as ‘ei in ligature with stroke through the descender of the i’, the Beneventan ti ligature for the assibilated sound, and the 2-shaped Beneventan interrogation sign, to which I would add the typical abbreviation for in as a long i cut by a horizontal stroke and the confusion of a and t. Interestingly enough, none of these features is found on fols. 66–95, those containing the new Consentius: from a codicological point of view, this is an autonomous section, written by a different scribe from the rest of the MS and preserving some grammatical texts generally attributed to insular authors, such as Smaragdus’ Liber in partibus Donati and part of the compilation entitled Pauca de barbarismo, which precedes the De barbarismis et metaplasmis; not surprisingly, the new text of Consentius displays numerous features of the Insular script, such as the symbols for enim, autem, eius, est, nihil and et. On this basis it is most likely that this whole section was never included in the Beneventan exemplar, but was added at the time and place of copying of our MS in order to enrich the grammatical content. Alchemy Tried in the Fire: Starkey, Boyle, and the Fate of Helmontian Chymistry. Chicago: University of Chicago Press, 2002. Pp. xv + 344. Cloth, $40.00. Newman and Principe have produced a masterful study of intellectual context, primarily by correcting the commonly held belief that there was a (...) radical break in the seventeenth century between late medieval alchemy and early modern chemistry. Although their main focus is on George Starkey (the 'alchemist') and Robert Boyle (the 'chemist'), they also provide copious details on the chemical practices and theories of Van Helmont, Benjamin Worsley, Frederick Clodius, and Kenelm Digby, among others. It is hard to do justice to their complex study in a short review. Following a brief summary, therefore, I will limit my discussion to how the authors' conclusions are significant for a more in-depth and nuanced understanding of philosophy in the seventeenth century.After a helpful introduction, Chapter 1 outlines the differences in the early lives of the English, aristocratic Boyle and the American, Harvard-educated Starkey. Chapter 2's background on late medieval chemistry includes an examination of corpuscular theories within that tradition as well as Van Helmont's arguments concerning the way in which mathematics is useful for explaining "superficial" changes but not the real, internal changes in bodies. Chapters 3 and 4 provide a close study of Starkey's laboratory notebooks. Among the entries, recorded in a style adapted from Scholasticism, there are passages that adopt the secrecy of alchemy, while others show Starkey's effective use of theoretical principles and such experimental quantitative methods as mass balance. Chapter 5 examines Boyle's intellectual context. Newman and Principe argue convincingly for Starkey's role in Boyle's addition of Helmontian elements to the general Baconian program. Chapter 6 looks at the [End Page 104] results of Boyle's eclectic mix of chemistry and mechanism. Although Helmontian chemistry played a significant role in Boyle's natural philosophy, the authors attribute his meager interest in quantitative measures and analyses to his attempts to use chemistry as a support for mechanism and as evidence against Aristotelianism. The concluding chapter draws together elements from previous chapters to provide the full argument against the idea of a radical break between medieval alchemy and modern chemistry.Newman and Principe's reconsideration of traditional categories surrounding the idea of a radical break should provide an impetus for similar reconsideration in the history of philosophy. As they point out, 'the' mechanical philosophy drew from a diverse range of disciplines. Boyle's eclectic and complex attempt to develop a unique version of the mechanical philosophy can be seen in other English philosophers, especially Locke and Newton. This study provides a promising basis for yet more extensive and refined studies of the seventeenth-century milieu. The continuing over-emphasis on the physical principles of Descartes and Gassendi's versions of the mechanical philosophy and the tendency to reduce all mechanism to their view needs revision. Also in need of reconsideration is the currently popular belief that the study of context simply means adding social elements to an uncontroversial intellectual context. As the authors clearly show, much more work on the intellectual context remains to be done.One line of research in the history of philosophy that could be pursued concerns the role of Francis Bacon in this context. Although Newman and Principe mention the general Baconian ethos, a detailed discussion of Bacon was clearly beyond their scope. Yet Bacon's own debts to chemistry, although frequently unrecognized, seem more significant given this study. Also, it would be interesting to see the extent to which Bacon's works influenced Van Helmont or Starkey. Newman and Principe's claims regarding the novelty of Starkey's notebooks, for example, may be a little overstated. A visitor to Oxford in November of 1837 gave the following picture of Newman's position: It was allowed that the Doctor [Pusey] and Newman governed the University, and that nothing could withstand the influence of themselves and their friends. Every man of talent who during the last six years has come to Oxford has joined Newman, and when he preaches at St. Mary's (on every Sunday afternoon) all the men of talent in the University come to hear him, although at the loss of their dinner. Aristotle's metaphysics has stimulated intense renewed debate in the past twenty years. Much of the discussion has focused on Metaphysics Z, Aristotle's fascinating and difficult investigation of substance , and to a lesser extent on H and Θ. The place of the central books within the larger project of First Philosophy in the Metaphysics has engaged scholars since antiquity, and that relationship has also been reexamined. In addition, scholars have been exploring the Metaphysics from various broader perspectives—first, in relation to (...) Aristotle's natural philosophy, his physics, biology, and psychology, and to the Organon, his so-called "logical" works, which include the Categories, Topics, and Posterior Analytics; and second, in relation to the broader philosophical tradition, both Plato before him and the ancient commentary tradition in late antiquity. Hempel's qualitative criteria of converse consequence and special consequence for confirmation are examined, and the resulting paradoxes traced to the general intransitivity of confirmation. Adopting a probabilistic measure of confirmation, a limiting form of transitivity of confirmation from evidence to predictions is derived, and it is shown to what extent its application depends on prior probability judgments. In arguments involving this kind of transitivity therefore there is no necessary "convergence of opinion" in the sense claimed by some personalists. The conditions (...) of application of the limiting transitivity theorem are most perspicuously described in terms of De Finetti's notion of exchangeability, which leads to a suggested revaluation of the function of theories in relation to confirmation and explanation. We hypothesize that effective collaboration is facilitated when individuals and environmental components form a synergy where they work together and regulate one another to produce stable patterns of behavior, or regularity, as well as adaptively reorganize to form new behaviors, or irregularity. We tested this hypothesis in a study with 32 triads who collaboratively solved a challenging visual computer programming task for 20 min following an introductory warm‐up phase. Multidimensional recurrence quantification analysis was used to examine fine‐grained (i.e., every 10 (...) s) collective patterns of regularity across team members' speech rate, body movement, and team interaction with the shared user interface. We found that teams exhibited significant patterns of regularity as compared to shuffled baselines, but there were no systematic trends in regularity across time. We also found that periods of regularity were associated with a reduction in overall behavior. Notably, the production of irregular behavior predicted expert‐coded metrics of collaborative activity, such as teams' ability to construct shared knowledge and effectively negotiate and coordinate execution of solutions, net of overall behavioral production and behavioral self‐similarity. Our findings support the theory that groups can interact to form interpersonal synergies and indicate that information about system‐level dynamics is a viable way to understand and predict effective collaborative processes. Reading the Shape of Nature vividly recounts the turbulent early history of the Museum of Comparative Zoology at Harvard and the contrasting careers of its founder Louis Agassiz and his son Alexander. Through the story of this institution and the individuals who formed it, Mary P. Winsor explores the conflicting forces that shaped systematics in the second half of the nineteenth century. Debates over the philosophical foundations of classification, details of taxonomic research, the young institution's financial struggles, and the (...) personalities of the men most deeply involved are all brought to life. In 1859, Louis Agassiz established the Museum of Comparative Zoology to house research on the ideal types that he believed were embodied in all living forms. Agassiz's vision arose from his insistence that the order inherent in the diversity of life reflected divine creation, not organic evolution. But the mortar of the new museum had scarcely dried when Darwin's Origin was published. By Louis Agassiz's death in 1873, even his former students, including his son Alexander, had defected to the evolutionist camp. Alexander, a self-made millionaire, succeeded his father as director and introduced a significantly different agenda for the museum. To trace Louis and Alexander's arguments and the style of science they established at the museum, Winsor uses many fascinating examples that even zoologists may find unfamiliar. Children show increasing control of emotions and behavior during their early years. Our studies suggest a shift in control from the brain’s orienting network in infancy to the executive network by the age of 3—4 years. Our longitudinal study indicates that orienting influences both positive and negative affect, as measured by parent report in infancy. At 3—4 years of age, the dominant control of affect rests in a frontal brain network that involves the anterior cingulate gyrus. Connectivity of brain structures (...) also changes from infancy to toddlerhood. Early connectivity of parietal and frontal areas is important in orienting; later connectivity involves midfrontal and anterior cingulate areas related to executive attention and self-regulation. In C.S. Lewis's science fiction parable Perelandra was a planet which had no solid ground. At all times the floating landscape was continually swirling and moving, chasms would appear where a minute before there had been safe standing. The rational beings who lived there hopped nimbly on to another little island when the one on which they stood disappeared under their feet. They were used to it and took it for granted that nothing was certain. The visitor from our planet (...) had to learn a completely new way of existence. But where did he get his idea of certainty in a fixed environment? It is more plausible that uncertainty is normal and the whole idea of certainty an illusion. Today gives an opportunity to reflect on how people deal with skepticism, doubt and uncertainty.The questions apply to a current debate in the UK on risk. Opinion polls constantly reveal that the public lacks trust in government, and particularly it does not trust the government to reveal the information needed to assess important risks. From which the risk analysts conclude that the government should grant access to information more freely and encourage enquiry. They believe that openness would foster a better understanding on the part of the public which at present does not know what to believe. More information would create certainty, more certainty and the public would trust its spokesmen, unreasonable fears would be calmed. This is the advice of an expert enquiry.Certainty is not a mood, or a feeling, it is an institution: this is my thesis. Certainty is only possible because doubt is blocked institutionally: most individual decisions about risk are taken under pressure from institutions. If we recognize more uncertainty now, it will be because of things that have happened to the institutional underpinning of our beliefs. And that is what we ought to be studying. In my student days the hottest controversies in anthropology were about why `other people' — that is people not living in advanced capitalist society — had certainty about their absurd beliefs.When trying to explain their misfortunes, why did they neglect the physical and scientific evidence, and draw instead on their beliefs in spirits, magic, and taboos? How could they be so obstinate in error? Anthropologists spent their energies on defending the allegedly irrational beliefs of other people, and I shall continue the tradition. (shrink)
